The “C” Sora Health District is located to the east of ASL Frosinone and includes the following municipalities: Alvito, Arce, Arpino Atina, Belmonte Castello, Broccostella, Campoli Apennino, Casalattico, Casalvieri, Castelliri, Colfelice, Fontana Liri, Fontechiari, Gallinaro, Isola del Liri, Monte San Giovanni Campano, Pescosolido, Picinisco, Posta Fibreno, Roccadarce, San Biagio Saracinisco, San Donato Val Comino, Santopadre, Settefrati, Sora, Vicalvi and Villa Latina with a resident population of about 100,000 inhabitants, largely elderly, spread over a vast territory, mainly mountainous, with an often insufficient road network and difficult climatic conditions.
The District represents the center of services and services where the demand for health is addressed in a unique and comprehensive way through:
- the provision of services rendered through the District Operating Units, ensuring integration between them, also in order to optimize the use of resources;
- the provision of services in the form of commissions through agreements and/or negotiations with the Managers of the Company’s facilities (Departments and Central Area Services, Hospital Facilities) or the purchase of services from accredited facilities, according to the indications of the Strategic Management;
- integration with Local Authorities for the identification of needs, the selection of interventions and the implementation of health promotion activities.

P.U.A. Single Point of Access
The Single Access Point is the place where every citizen can turn for information on territorial services ranging from the services provided, to the methods and times of access. It is aimed in particular at people with complex needs, especially the elderly and those with disabilities, and is managed by a staff of operators who listen to and analyse the needs of the user and their family members, identify with them the most suitable therapeutic path and accompany them at every stage of the response to these needs.
Access to territorial care takes place through the P.U.A. defined as an organisational modality agreed between the District and the Social Area, and has the following objectives:
- Guarantee Citizens and Operators a unitary gateway to access the social and health services of the territory
- Guarantee a widespread informative action on care, social and social-health care pathways also through the activation of a toll-free number
- Guarantee the submission to the UVMD for the assessment of the need and for the possible taking charge
- Guarantee the continuity of the care path
- Ensure simplification of administrative procedures
The P.U.A. can be activated, through different modes of communication, by:

U.V.M.D Multidimensional Evaluation Unit District
The UVMD, unique for the District and made up of a multidisciplinary team of health and social professionals coordinated by the District Director or his delegate, is responsible for the multidimensional assessment of the user’s need with the use of the assessment tool identified by the Region for access to health and social health services and in particular:
- access to rehabilitation benefits pursuant to art. 26 L.833/78 (outpatient, residential, semi-residential, extramural and domiciliary).
- access to residential and semi-residential psychiatric facilities (subject to authorization by the Director of the Department of Mental Health for Adults or the Director of the Maternal and Child Department for Minors).

Card authorisation for patients with celiac disease

To authorise the purchase of celiac disease products, it is necessary to present, at the Health Centre of Isola del Liri, Via dell ‘Ospedale
- medical certification attesting to the disease;
- celiac disease exemption code issued at the exemption offices of District “C” ;
- authorization of their health card with the issue of a pin code, in order to deliver the products to the authorized centers (pharmacies, supermarkets, etc.).

Authorization of Medically Assisted Reproduction services

The authorization to use the services at affiliated private facilities outside the Lazio Region, is granted , at the request of the interested party, at the expense of the Region of residence, requires 30 working days for the completion of the planned investigation and subsequent authorization, as it is possible that, during the processing of the application, further documentation may be necessary to be produced for any further investigations.
Current legislation and, finally, Regional Law no. 19 of 23.11.2022 establishes the criteria and methods for access to WFP techniques at the expense of the SRH , in particular:
- for homologous assisted fertilization, the maximum age of the woman is set at 43 years of age;
- for heterologous assisted fertilization by donation of male gametes, the maximum age of the woman is set at 43 years of age;
- for heterologous assisted fertilization by donation of female gametes, the maximum age of the woman is set at 46 years of age.
Presentation mode
In order to accept the application for authorisation, the following documents must be attached to the application form, issued by the facility where the service is carried out:
- Certification of a specialist doctor in Obstetrics and Gynecology or Endocrinology with andrological skills or in Urology with andrological skills or in Medical Genetics of the center chosen by the couple, signed, dated and stamped, certifying the state of infertility or sterility and related indication to proceed with WFP techniques according to current legislation.
- Certification of the WFP techniques provided by the chosen centre and related estimate, with planning of execution times that must take into account the necessary 30 working days from the date of submission of the application to receive any required authorisation.
- Authorisation for the processing of personal data by the couple, Articles 13 and 23 of Legislative Decree no. 196/2003.
- Self-declaration of the number of total cycles carried out by the woman over time, indicating the date and place of execution, borne and not by the National Health Service (S.S.N.)
- Copy of the identification document of the interested parties.
