Services/Activities provided
The Services provided by the Basic Health Care of the District A Anagni-Alatri are as follows:
- Specialist outpatient clinic;
- Choice and Revocation of the Physician;
- Ticket exemptions for income and pathology;
- MMG / PLS / Continuity of Care supervision;
- Dispensing of food for rare diseases;
- Organisation of the Influenza and Pneumococcal Campaign;
- Renewal of Health Card;
- Activation of Health Card with service card and electronic health file;
- Recipe management
Users who must request the Choice of a new Family Doctor/Pediatrician, the Revocation of the assigned Doctor, the European Sickness Insurance Card (TEAM), the Electronic Health Card (TS-CNS), the Self-certifications for Income, not present in the ASUR system, residing in the territorial areas of the “A-B” Districts (Anagni-Alatri-Frosinone) can send requests by email to the email address sportelloscelta.ab@aslfrosinone.it.
The documents to be forwarded by e-mail are as follows:
- copy of the identity document and copy of the health card
- in the case of persons appointed by third parties, proxy of the delegating party with a copy of the identity document and a copy of the health card of both (delegate and delegator)
The forms in use are available on the institutional website www.asl.fr.it New ways of accessing Choice/Team/TS-CNS/Income Exemptions services.
It is understood that this request is limited to users residing in the territorial areas of the “A-B” Districts (Anagni-Alatri-Frosinone).
These services are offered to all citizens belonging to District A, including non-EU citizens of the territory, specific groups affected by rare diseases.
How to access
There are two types of access methods:
- Direct access;
- CUP BOOKING
Reception times and days District A
The days and times of receipt are as follows:
- Every day including Saturday;
- From Monday to Friday from 8:30 am to 12:00 pm;
- Tuesday and Thursday from 3:00 pm to 5:00 pm
Reception times and days District B – Frosinone
The Medical Choice Desk/ Exemption/Team / CNS service will be open to users on the following days:
- Monday and Tuesday from 8:30 am to 12:30 pm;
- Wednesday, Thursday and Friday from 13:30 to 17:30
