The activity of Integrated Home Care is aimed at people suffering from chronic diseases, clinical and functional syndromes, multiple deficits that result in disability and non-self-sufficiency which, for the most serious cases, after a multidimensional evaluation, may provide access to residential or intermediate facilities: RSA, Long-term Care, Hospice and Day Centres, in integration with the social services of the Municipalities.
