Company Procedure no. 8/2022 – Rev. no. 00 of 16.03.2022
Pursuant to the Prime Ministerial Decree of 4/2/2022, the certification of exemption from vaccination against Covid 19 is issued, at the request of the interested party, in cases where the vaccination itself is omitted or deferred due to the presence of specific documented clinical conditions that contraindicate it permanently or temporarily, as established by the Circular of the Ministry of Health 35309 of 4/8/2021 and subsequent amendments; the same is included in the Health Card System for digitisation, starting from 01.03.2022.
The Lazio Region with note prot. U.0164273 of 17/02/2022 established the procedures for issuing these certifications, on the basis of which the following company procedure has been defined and activated.
Who can issue the exemption certification in the ASL FR
- General Practitioners (GPs) or Paediatricians of Free Choice (PLS) participating in the Covid-19 Vaccine Campaign at their own practice/clinic, as per the list filed in ASL, limited to their patients;
- Doctors participating in the Covid-19 Vaccine Campaign at the ASL FR Vaccine Centers, depending on the different contracts in place, for patients assisted by MMG/PLS not adhering to the Campaign or for patients without MMG/PLS assistance.
Who can apply for exemption to the ASL FR
- Users resident/domiciled or entitled in the territory of ASL Frosinone;
- Parents/Guardians of minors residing/domiciled or entitled in the territory of ASL Frosinone.
To whom to apply for exemption
- Users residing/domiciled or entitled in the territory of ASL Frosinone or Parents/Guardians of minors residing/domiciled, or entitled in the territory whose General Practitioner (GP) or Paediatrician of Free Choice (PLS) has joined the Covid Vaccine Campaign, must apply directly to their GP/PLS;
- Users residing/domiciled or entitled in the territory of ASL Frosinone and Parents/Guardians of minors residing/domiciled, or entitled in the territory, whose General Practitioner (GP) or Paediatrician of Free Choice (PLS) has not joined the Covid Vaccination Campaign or who are in the absence of MMG/PLS, may apply by contacting the ASL Vaccination Offices.
How to apply to the ASL FR
- Users may apply for an exemption for the anti-vid-19 vaccination at the ASL FR, by submitting the following documentation to the ASL Vaccination Centre:
- Application form for adults (Annex 1, Annex and downloadable from the ASL FR website) or application form for minors or persons under guardianship (Annex 2, Annex and downloadable from the ASL FR website) duly completed;
- Copy of reports and examinations useful for evaluation purposes;
- Copy of the valid identity document, dated and signed, as indicated on the form;
- Copy of the front/back health card, as indicated on the form;
- Copy of any previous paper exemption.
- The request will be acquired by the ASL Vaccinating Physician on duty at the Vaccination Center, signed for acceptance and submitted for evaluation;
- In case of acceptance of the application and issuance of the exemption, it will be included in the Health Card System (TS) for digitization by the Certifying Vaccinating Physician who has taken charge of the request and is enabled for the specific function of the TS system
The ASL may accept or reject the request or request further diagnostic investigations deemed necessary for the purposes of the evaluation.
Second Level Fee
In compliance with the provisions of the aforementioned regional note, a second-level Company Commission is also established in the ASL FR for the collective evaluation of any doubtful situations related to ineligibility for vaccination in support of Certifying Doctors.
In case of doubt for particular situations, Certifying Doctors (Vaccinating Doctors ASL/MMG/PLS) may request the opinion of the aforementioned Commission by sending a request, by email, to the dedicated address: propuestaesenzione.vaccinocovid@aslfrosinone.it, attaching all the documentation acquired and useful for the purposes of the evaluation.
The Commission will express the opinion and will communicate it, by the same means, to the Certifying Physician to support the evaluation process and the decision of acceptance or refusal.
The Certifying Physician, having obtained the opinion of the Company Commission, will complete the procedure.
N.B.: Subjects who obtain an exemption to SARS-CoV-2 vaccination must be adequately informed about the need to continue to maintain appropriate prevention measures such as:
- use the masks according to the indications provided for by the regulations;
- maintain suitable distancing from non-cohabiting persons;
- wash your hands frequently;
- avoid gatherings, in particular in closed rooms;
- comply with the conditions provided for by the regulations for workplaces and means of transport.
The exemption certificate is issued free of charge
