We inform all those who need to issue a Green Pass for Covid-19 Healing that the Certificate of Healing must be issued by the General Practitioner who must enter the data of their patient on the Health Card (TS) system so that the system starts the automatic procedure for issuing the Green Pass.
Persons who for any reason do not have health care in the Lazio Region (residents outside the region, tourists, etc.) and who have contracted Covid during their stay in our Province, can contact the SISP (Services of Hygiene and Public Health) Companies for the certificate of healing through the TS system and issuance of the Green Pass by sending an email to the address: sisp.direzione@aslfrosinone.it, attaching:
- copy of the Health Card (for those who are in possession of it);
- copy of the Identity Document (for persons without NHS health care);
- copy of the First Buffer Positivity Report for COVID-19;
- copy of the COVID-19 swab Negativity Report carried out at the end of isolation (not necessary for subjects still positive after 21 days from the first positive, if asymptomatic for at least seven days and not immunocompromised);
- self-certification stating that the documentation sent belongs to the applicant and/or the minor relatives.
