In order to ensure the exercise of the right to vote by voters in conditions of home isolation for COVID-19 (Legislative Decree 04.05.22 n.41 converted into Law 30.06.2022 n.84) the voter must send, among other things, to the mayor of the municipality in whose lists he is registered, a certificate issued by the medical officer designated by the competent bodies of the local health company attesting to the existence of the conditions provided for by the decree law for the right to vote at home (home treatment or conditions of isolation for Covid-19).
For the above, interested parties may request the issuance of the aforementioned certification to the territorially competent SISP (Hygiene and Public Health Services) by sending an email to one of the following addresses:

sisp.distrettoc@aslfrosinone.it for all municipalities included in the territory of District C ASL FR
sisp.distrettod@aslfrosinone.it for all municipalities included in the territory of the District D ASL FR
The email must contain the words: “Covid-19 certification request for electoral use” and in the text of the email the requesting party must report the following data:
- surname, first name, date of birth and current address.
In the email itself, a copy of the identity document and a copy of the Covid-19 positive report will be attached.
Requests received by email will be taken over by the SISPs, in the manner described above, until 4.00 pm on 25.09.2022.
